Child(ren)'s Information 

EMERGENCY CARE AND RELEASE OF LIABILITY

I give permission for my child(ren) (above), to attend the Summer Oratory Camp 2026 at Sacred Heart Cathedral in Fairbanks, Alaska. In consideration of my child(ren)’s permission to participate in the camp, I personally and on behalf of my child(ren), hereby release and forever discharge the faculty, staff, coordinators and chaperones of Summer Oratory 2026, the Diocese of Fairbanks, the Institute of the Incarnate Word, the Priests of the Institute of the Incarnate Word, the Institute Servants of the Lord and the Virgin of Matará, Inc., and the Religious Sisters of the Institute Servants of the Lord and the Virgin of Matará from any liability for injuries or damages of any kind arising or resulting from my child(ren)’s participation in the camp. In the case that I cannot be reached, I hereby grant permission for my child(ren) to be evaluated, diagnosed, treated and/or medicated in accordance with standard medical practice by licensed medical personnel.

CODE OF BEHAVIOR AGREEMENT

Likewise, my child(ren) agree(s) to obey all the rules expressed in the Code of Behavior. I agree that the diocesan volunteers at the Summer Oratory, the Diocese of Fairbanks, the brothers and Fathers of the Institute of the Incarnate Word and the Sisters Servants of the Lord and the Virgin of Matará will not be held liable if my child does not comply with the aforementioned Code of Conduct and that any infraction of the Code could result in the immediate expulsion of my child(ren) from the camp. I will be responsible for any fees incurred resulting from their immediate transportation home.

PHOTO CONSENT

Likewise, I authorize and give full consent, without limitation or reservation, to the Catholic Diocese of Fairbanks, the Institute of the Incarnate Word, and the Servants of the Lord and the Virgin of Matará to publish in productions, publications, on the web, or other printed or electronic materials any photographs or videos in which the named child(ren) appear(s) while participating in the Summer Oratory 2026. There will be no compensation for use of any photographs at the time of publication or in the future.
