Photos and videos will be taken during the Family Gathering. We would like to be able to use these photographs and videos for parish and diocesan publications and websites. Names will not be published unless written permission is given by the participant and parent/guardian, and only the first name will be used. If there are concerns about photos or videos posted online, please contact the parish office and they will be promptly removed.

I/We, the parents/guardians of this family, hereby authorize and grant our consent, without limitation or reservation, to the Institute of the Incarnate Word, the Servants of the Lord and Our Lady of Matará, Immaculate Conception Parish, and the Diocese of Fairbanks, to publish any photographs or videos featuring my family as we participate in the Day of Families. There will be no compensation for the use of any photograph or video at the time of publication or in the future.
